The Imperial College of Surgeons is an Imperium-wide body administered by the Ministry of Health, whose primary duty is to set consistent standards of conduct for practicing Surgeons.
As part of it's charter, the College must maintain the list of members; is allowed the right to set standards and is permitted to admit persons meeting those standards to membership through examination. It has the right to determine the criteria and procedure for removing persons from that list, excepting only that a person must be removed as the result of the Emperor's expressed displeasure.
Examinations are given annually at several locations in each sector, and consist of a series of written tests and an oral examination by a board of Surgeons who are existing members and observation of the applicant-Surgeon at work. Tests are given for various specialties. The tests ensure that the person has an adequate medical skill and appropriate dexterity to undertake the work of Surgery.
Purporting to be a member of the College (when one is not so in fact) is a Treason because membership of the college potentially grants the person access to the body of the Emperor.
Individual worlds often have their own College of Surgeons and certification procedures, but these do not grant automatic certification on any other world. The Imperial College certification is valid on any-world in the Imperium.
Most established University Hospitals will provide post-graduate courses to prepare students for the Imperial College of Surgeons examinations.
Curiously, there is no bar on any person declaring themselves to be a Surgeon within the Imperium, although most planetary jurisdictions will have some minimal requirement to validly accept patients. However, only Members of the Imperial College may practice within the Imperial Palace, a restriction echoed on many Noble estates.
To enjoy the title of 'Ship's Surgeon', the person concerned must be certified by the College of Surgeons of the ships port of registry or a College recognised by that port's planet, or must be a member of the Imperial College of Surgeons.

The Imperial College of Surgeons was established by Cleon I when he wished to ensure that the Surgeons operating within the Imperial Palace were qualified. It creates the body as a corporation and grants its members various rights.
