5660AD. The Provincial Merchant Kahiik Khi suffered thruster failure and collided with the outer hull of the Tiik'sch Waypoint, causing a breach and partial loss of internal atmosphere. Pressure doors and safety equipment failed to work and 72 lives were lost. The Kahiik Khi had only minor damage.

The subsequent investigation revealed that repeated fixes, workrounds and repairs, done to keep the station running over centuries of use, had compromised almost all safety protocols. Retrofitted ducting and cables had prevented internal bulkhead doors from closing, test procedures had been overridden or removed and hydrogen containment throughout the station presented a substantial hazard. The situation was little different on the Sch'ikaa Waypoint
