The legal entities that are responsible for find that the risk of injury or damage to third parties, and the associated litigation, is a risk that is existential. i.e. should some disaster occur, the legal entity would effectively be financially ruined. As a consequence both the legal entity, and those using the services of that legal entity will tend to insist on appropriate insurance - the former to protect their ability to continue to operate; the latter to ensure that should some catastrophe occur, the damages that may occur can be rectified by that entity.
When the ports insist on insurance before they will allow approach, lack of insurance also starts to be an existential issue for shipping and insurance starts to become ubiquitous.
In the New Earth System, all the major planets will seek proof that ships using its ports are insured. Some will decline to legally allow passengers aboard an uninsured ship, or refuse an uninsured ship legal access to their airspace.
